Due to the outbreak of corona virus we have suspended online and minor illness walk-in appointments. If you need to see a GP please telephone reception where a GP will phone you back if needed. TO PROTECT YOURSELF AND OTHERS DO NOT COME TO THE SURGERY UNLESS A GP HAS GIVEN YOU AN APPOINTMENT.
If you have an appointment pre-booked with the surgery within the next 4 weeks, we will contact you 24 hours before your appointment to confirm if you are well to attend
Change to opening hours from 1st April. Wrose Surgery and Haworth Rd Surgery will open 8.30am until 1pm Mon to Fri. Leylands Medical Centre and telephone lines for all surgeries will be open from 8.30am until 6pm Mon to Fri. Please do not attend any surgery unless agreed by a GP or Nurse

Please use the following service if you have to stay at home because of coronavirus and you need a note for your employer.
If you have to stay at home but feel well enough to work, ask your employer if you can work from home. If you can work from home, you will not need an isolation note.
Please note, medication can still be ordered using the usual methods; Online, via the NHS App and Via E-Consult
if at all possible, please do not come to the surgery in person to request medication.
Extended Access Appointments
From the 3rd April 2018 this practice is taking part in the GP extended access service which is provided from three sites in Bradford.
This means you will offered the opportunity to attend an evening appointment (after 6.30pm) or a weekend appointment at one of the three sites to see a healthcare professional or to take part in other activities that will improve your health and wellbeing.

Welcome to Leylands Medical Centre. Our aim is to provide high quality care for all our patients. PPG Survey 2020
The Patient Participation Group (PPG) for Leylands Medical Centre (covering Leylands, Wrose and Heaton Surgeries) is asking for your help in carrying out the annual survey of the Practice's services. The survey will take place over a three-week period, commencing on February 17th until March 6th 2020.
